• Is my insurance plan self-funded?

• Are ABA benefits covered under my medical benefits or mental health benefits?

• Does my ABA provider have to be "in-network"?

• Does my provider need to bill with an authorization number?

• Must ABA therapy claims be billed with an ICD-9 of F84.0?

• Will my cost-share for ABA therapy be a co-payment or co-insurance?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• What is the coverage amount per therapy session?

• Is a referral required from my primary care physician?
